When you put on the lens you have to check to see if the locking tab, locks into the lens locking tab as shown below.

Now to release the lens make sure to pull the release knob straight up. 

Make sure the release knob does not tilt when pulling up on the knob as shown:

We've usually had luck with removing this adapter by placing the camera mount side on a table or other flat/sturdy surface.

Hold the release knob (highlighted in red) down and hold. The portion highlighted in green does not move, but the portion highlighted in yellow needs to be rotated in the direction of the yellow arrow.
